Established in 2004, LakeMed’s Clinical Practice has been consciously designed to align the provision of primary health services in our community with our ideal standard of care. The Medical Home is the central hub for the timely provision and coordination of a continuous and comprehensive health and medical services for children, youth and adult and with the goal of obtaining maximized health outcomes. This model will provide the patients with timely access, personalized health care, comprehensive medical services and continuity of care. It is a strong component of a healthy family and community.
